Referral criteria for care at home

Patients can be referred for care at home for the following reasons:

  • Assessment and management of complex unstable symptoms
  • Support and information for patients and carers
  • Support for professionals caring for patients with advanced illnesses or at the end of their lives
  • Assessment for in-patient symptom management and end of life care

Referral criteria for in-patients

Patients who are admitted to the Hospice In-Patient Unit generally fall into the following categories:

  • Symptom management – many of whom are then able to return home
  • Psychosocial crisis – where the situation at home has become untenable – these admissions vary in length, while we support people to resolve the problem, or move to their preferred place of care
  • Palliative rehabilitation – providing intensive support to help people maintain their quality of life
  • End of life care

If you would like to discuss a referral, please contact our Clinical Admin Team on 0300 10 20 100 and press option 1.
