Most of our patients choose to be cared for in the familiar surroundings of their own homes. Our Hospice at Home services help make that possible.

By bringing our expert, compassionate care to the people who need us where they feel most comfortable, we help reduce hospital admissions and relieve the strain on other health providers. Our different teams work together to bring seamless care to local people in the areas we cover.

Who we are

Hospice at home – A range of healthcare professionals including nurses, doctors and therapists who are available during the day, with nurses on call until 9pm. They plan each person’s care and give advice to patients and their loved ones.

Rapid response – Palliative care registered nurses respond to calls and make urgent visits to people in their own homes during the day.

Night response team – Specially trained palliative nurses and healthcare assistants who give advice over the phone or make home visits to patients from 10pm. Provided by us in Surrey Downs and Kingston, and by District Nurse teams in other areas.

Enhanced support – This team provides specialist care and support to local patients, who have rapidly deteriorated but want to remain in their own homes. They work together to address the patient’s needs and wishes.

Delivering free compassionate care across 600 square miles

“We cover a huge area. If somebody rings and they’re urgently needing help we try and get to them as soon as possible, but often it’s not easy geographically – you have to be able to think on your feet. Even when you’ve decided to go to one call, something else might come in and you need to reroute and communicate what’s happening and how long it’s going to take.”  – Camilla, Specialist Palliative Nurse, Princess Alice Hospice Night Response Team
