Community Palliative Care Team

Our community team offers specialist advice and support to patients and their carers at home, in nursing or residential homes, hospitals or anywhere our expertise can be helpful in dealing with the physical, emotional, social or practical difficulties that people face when they have a life limiting illness.

The community Clinical Nurse Specialists (or doctor) will usually meet the patient and their carers at home but they can be seen at a clinic, hospital or the Hospice. Clinical Nurse Specialists liaise directly with hospital consultants and their teams, General Practitioners and District Nurses to ensure continuity of care.

The community team provides a 7 day visiting service and a 24 hour on-call advice and support service each evening and night.

The team also run a weekly Carers Support group for carers of patients from any of the Hospice services.
